Arizona Medical Board Provides Information on Advance Care Directives
 

Scottsdale, Ariz. - In light of the issues raised by the Terry Schiavo case in Florida, the Arizona Medical Board has assembled information on Advance Care Directives for both physicians and consumers. The material is now available here on the Board‘s web site.

The information for consumers is located in the “Consumer Center” area. Living Wills and a Durable Health Care Power of Attorney are explained. There is an Internet link to the Arizona Attorney General‘s web site where forms for both can be downloaded and printed. Another link can take consumers to the Arizona Secretary of State‘s web site where they can register their Advance Care Directives at no charge.

The Advance Care Directive information for physicians can be found under the menu heading, “Physician Center.”
